Geontological care channels in Auvergne-Rhône-Alpes


Presentation of gerontological sectors

Since 2010, gerontological sectors (FG) have been set up in the former Rhône-Alpes in order to structure a more fluid and effective care pathway for the elderly. As for the ex-Auvergne, this coordination function is notably organized by the actors of the intermediate health pools (BSI)

Conceived as coordination spaces between the actors of the health, medico-social and social, these sectors aim to facilitate the interconnection of professionals and to limit breaks in the support of seniors. After more than a decade of operation, an inventory produced by Alcimed in 2023-2024 at the request of the Regional Health Agency (ARS) Auvergne-Rhône-Alpes, highlights the general relevance of these sectors while targeting disparities and areas of improvement, as well as the prospects to be given to the regional model. For this, the Alcimed group has carried out territory monographs by department identifying the operation and organization of each of the gerontological sectors.

Role and missions

A key role in supporting aging

The gerontological sectors (FG) were created to meet the challenges of aging, ensuring better monitoring of elderly patients, promoting their home support and avoiding unnecessary hospitalizations. They also make it possible to strengthen network work between the various actors (hospitals, nursing homes, home services, local communities, etc.) and to develop a common gerontological culture. The FGs are also beneficial to the interconnection between actors on the ground but also institutional (departmental councils, departmental delegations) involved in the care of the elderly.

Their action is based on several essential missions:

  • Fluidify the care path of the elderly.

  • Ensure better coordination between professionals.

  • Develop prevention tools and actions.

  • Strengthen the training and awareness of actors.

  • Promote interconnection between the actors of gerontology of the same territory

  • Fluidify the course of the elderly

  • Facilitate return home and avoid hospitalizations and emergency passes

  • Build a common gerontological culture and disseminate it.

The sectors have seized their missions by adapting them to their territories (organization of events and training, creation and the provision of tools)


The gerontological sectors: how does it work?

When they were created, all the sectors were thought of according to the same organizational model promoting the meeting between the actors in the elderly’s journey:

  • The financing by ARS Auvergne-Rhône-Alpes of a part of a part-time sector assistant by sector.

  • Intervention territories built around a living area,

  • A decompartmental and representative organization of the actors,

  • An objective of network work development to meet the needs of the elderly by avoiding hospitalizations and allowing a fluid return to home.

  • A co-animation carried by the local hospital center (CH) and the Departmental Council through support for coordination support (DAC).

Regional management is ensured by an inter-channel committee bringing together all the sectors as well as representatives of ARS Auvergne-Rhône-Alpes. Departmental support is also provided by the departmental delegations of ARS Auvergne-Rhône-Alpes and the departmental councils.
