Presentation
A 51 -year -old patient is admitted to the emergency room for dyspnea. It presents as a particular history of arterial hypertension (HTA) treated and type 2 diabetes under Metformin alone. He does not take any other treatment and does not present an allergy. He has described a crescendo dyspnea for a month, currently Nyha 4 with orthopnée, and presents francs edema of the lower limbs.
Its constants are as follows:

blood pressure: 110/81 mm Hg

Heart frequency: 108 BPM
-
SPO2: 95 % under 4L/min of oxygen

temperature: 36.2 ° C
Exams
The clinical examination finds crackling of the two bases without a franc home, and a mitral systolic breath.
The chest x -ray finds bilateral interstitial syndrome with minimal right pleural effusion.
An ECG is made (Figure 1).
