Safety system in CHSLD | A man run away from a closed unit

At 5:11 a.m. on June 21, Serge Dumas, a 77-year-old man suffering from dementia, running away from the manor-trinity, where he resides in a closed unit. The accommodation center, like all CHSLDs and house houses, is however equipped with security systems.

“If we want to get out of my father’s unity, it takes a code to open the door. Then it takes a code in the elevator, and to go out outside, it is someone who has to open the door, ”explains Mélanie Dumas, the daughter of the resident.

To mme Dumas, the management of the CHSLD said that the resident had passed through an unleashed outing and found himself in the parking lot of the establishment, where he fell. Note that the accommodation center is located on the edge of the Jacques-Cartier boulevard, a passing artery.

Half an hour later, an employee arriving at her workplace would have found her extended on the parking floor.

A person from the CHSLD would have said to Mme Dumas that because of the small number of employees at night, the doors remain unlocked and open.

“I was not aware of that, I was informed,” said Mr.me Dumas in an interview. I had the perception that the door was crossed at all times because they are patients at risk of wandering. »»

Her father has always worn his security bracelet, said to The press CISSS Montérégie-Est, head of the establishment. It is because of a mechanical door defect that the alarm did not trigger. The mechanism was repaired within 24 hours of the incident, it was said to The press.

The bracelet acts an alarm if the person crosses a door or the elevator when it is not authorized.

A second version of the facts

The president of the CISSS workers’ union, affiliated with the CSN, Luc Michaudville, offered a completely different version of the facts after consulting its members about the incident.

According to him, the care team withdrew the resident safety bracelet “because it was no longer necessary”.

Mr. Dumas would have left the establishment by an unleashed outing without being noticed. Without a safety bracelet, no door prevented him from going out.

He then continued on his way to the Pierre-Boucher hospital, located about 200 meters away. At that time, the security of the hospital would have called the CHSLD to inform him of the presence of Mr. Dumas.

The CHSLD employees would have tried to reach the management of the establishment at the same time, but it would have taken 15 minutes to give directives: “For me, 15 minutes of waiting in the event of a problem, it is unacceptable”, deplores Mr. Michaudville. The man would have tripped in parking on the way back, accompanied by staff.

It was only four hours after the event that Mme Dumas received the phone call from a nurse to inform her.

“No one had access to the cameras and could confirm me how my father found himself outside and especially how long. The answer was: “it’s going to go on Monday”, deplores Mme Dumas.

On June 23, she received a call from Kossounou Kouadio Kra, chief of unit with mansion. The latter explained to him the chronology of events with the hours recorded at the surveillance cameras and the measures put in place by management.

Loss of confidence

In 2023, the father ofme Dumas lived in a RPA on the Montreal South Shore. Following several unopensed outings, it was moved to CHSLD to obtain a larger level of care. After the June event, Mme Dumas is not sure that his father is more safe in the accommodation center.

“All the management of this situation made me completely lose confidence in this system,” she said. According to her, the protocols are applied with little empathy and listening.

The situation experienced by Mr. Dumas is an isolated case, according to the Cisss Montérégie-Est and the union. There was no lack of staff that night, even if the night workforce was reduced.

In Quebec, “85 % of people hosted in CHSLD have a major neurocognitive disorder, which makes these people at risk of error,” said the Ministry of Health and Social Services by email on Monday.
